Lasting Power of Attorney (LPA) Review

$54.50

You can now get your Lasting Power of Attorney (LPA) witnessed and certified by doctors from Raffles Medical.

An LPA is a legal document that allows a person (the “donor”) to appoint one or more persons (the “donee(s)”) to make decisions and act on their behalf if they lose mental capacity. The donor must be at least 21 years old and have the mental capacity required to create an LPA.

This document must be signed by a Certificate Issuer – an accepted medical practitioner (doctor), a practising lawyer, or a registered psychiatrist – who will certify that the donor understands the implications of making an LPA.

Raffles Medical offers a panel of doctors who serve as LPA Certificate Issuers.

Benefits of a LPA

  • Allows a trusted person of your choice to act on your behalf over personal welfare and/or property & affairs matters, should you lose mental capacity
  • Provides peace of mind for yourself and your loved ones should you lose mental capacity

The Office of the Public Guardian (OPG) has extended the LPA Form 1 application fee waiver for Singapore Citizens to 31 March 2026.*

Description

Creating a LPA not only gives you the freedom to choose who will make decisions on your behalf, you will ensure you and your loved ones' interests are protected should you lose mental capacity.

Before selecting your donee(s), ensure that:

  • Your donee(s) is trustworthy and competent.
  • You have decided the decisions you wish to empower your donee(s) to make on your behalf.
  • You have set in place any limitations or safeguards on your donee(s).

Without a LPA, your loved ones may have to seek a court order to appoint someone to manage your affairs should you lose mental capacity.

To make an LPA:

  1. Choose your Donee(s) and decide what decision powers to grant your Donee(s).
  2. Draft your LPA online.
  3. Visit a Certificate Issuer (CI) to certify your LPA and sign using Singpass digital signature.
  4. Donor and Donee will be notified of LPA status via SMS / email when the digital LPA is accepted and ready.
