Lasting Power of Attorney (LPA) Review 2019
You can now get your Lasting Power of Attorney (LPA) witnessed and certified by doctors from Raffles Medical.
A LPA is a legal document which allows a person (known as the “donor”) to appoint one or more persons (known as the “donee(s)”) to make decisions and act on his behalf if he loses mental capacity. The donor should be at least 21 years old and have the necessary mental capacity to create a LPA.
This document needs to be signed by a Certificate Issuer -an accepted medical practitioner (doctor), a practicing lawyer, or a registered psychiatrist – who will certify that the donor understands the implications of making an LPA.
Raffles Medical offers a panel of doctors who are LPA Certificate Issuers.

For the LPA to take effect, you will have to submit your application form, together with the letter of certification by a LPA certificate issuer to the Office of Public Guardian (OPG) by post.
