Male Fertility Basic

For this package, you will need to come twice:

1. To do the semen analysis:
   • 8:30am to 2pm on weekdays
   • 8:30am to 10am on weekends
2. After the results are ready, you will see our male fertility specialist to discuss the semen analysis and treatment plan (if any).

This can be scheduled 2 to 3 days after the semen analysis or at the next available appointment.

Consultation hours:

• Monday to Friday: 9am to 12:30pm
• Tuesday and Wednesday: 2pm to 5pm

Kindly provide the following details for appointment booking:

1. Full Name (as per NRIC/FIN) 
2. NRIC/FIN Number 
3. Date of Birth 
4. Nationality 
5. Allergies (if yes, please state the symptoms) 
6. Contact Number 
7. Email Address 
8. Preferred Date and Time for semen analysis 
9. Reference Number: (where to find?)

Important Reminder:
There should be no sexual activity involving ejaculation for at least 2 days before the test. However, you should not avoid ejaculation for longer than 5 days.

• Price is not inclusive of additional tests and follow-up consultations recommended by the specialists.
