This service is for those renewing their Vocational Licence, or Private Driver Vehicle Licence (PDVL).

The service includes:

Physical examination by a doctor
Clinical measurement: Visual acuity
Clinical measurement: Blood pressure

Please bring along the following for your checkup:

Identification documents
Renewal notice/medical notification letter
Medical examination form
Specialist memo (if you have any pre-existing medical conditions)
Prescription spectacles

Please bring along a copy of your redemption email (either in hard or soft copy) and the above-mentioned items on the day of your appointment.
Additional test items will be charged separately.
Items in the service are non-exchangeable and non-refundable, including unutilised portion.
You may transfer the service to another person. Please fill in the authorisation form accordingly.
Not valid with other corporate programmes.
