Lasting Power of Attorney (LPA) Review 2019
You can now get your Lasting Power of Attorney (LPA) witnessed and certified by doctors from Raffles Medical.
A LPA is a legal document which allows a person (known as the “donor”) to appoint one or more persons (known as the “donee(s)”) to make decisions and act on his behalf if he loses mental capacity. The donor should be at least 21 years old and have the necessary mental capacity to create a LPA.
This document needs to be signed by a Certificate Issuer -an accepted medical practitioner (doctor), a practicing lawyer, or a registered psychiatrist – who will certify that the donor understands the implications of making an LPA.
Raffles Medical offers a panel of doctors who are LPA Certificate Issuers.
Benefits of a LPA
- Allows a trusted person of your choice to act on your behalf over personal welfare and or property & affairs matters, should you lose mental capacity
- Provides peace of mind for yourself and your loved ones should you lose mental capacity
Available at selected Raffles Medical (Singapore) clinics. Appointment is required.
Upon purchase, we will send to your email a redemption letter that you can use to redeem your service. Please call your preferred clinic to make an appointment and present the letter on the day of your appointment.
Creating a LPA not only gives you the freedom to choose who will make decisions on your behalf, you will ensure you and your loved ones’ interests are protected should you lose mental capacity.
Before selecting your donee(s), ensure that:
Without a LPA, your loved ones may have to seek a court order to appoint someone to manage your affairs should you lose mental capacity.
For the LPA to take effect, you will have to submit your application form, together with the letter of certification by a LPA certificate issuer to the Office of Public Guardian (OPG) by post.
